00:00The inadvertent extension of the lift dump spoilers was the primary cause of the plane
00:06crash near Bandar Almina, Siaalam last year.
00:11In its final report on the crash, the Air Accident Investigation Bureau stated that
00:16the sudden loss of lift resulting in catastrophic loss of control and the subsequent crash was
00:21believed to have been caused by a fatal error made by the co-pilot.
00:26The report also highlighted a number of other contributing factors that led to the crash
00:30which killed 10 people, including Pahang Executive Counsellor and Pelangang Assemblyman Johari
00:35Harun.
00:36It said the flight crew were sitting in the wrong seats, which would have likely contributed
00:40to ineffective crew resource management and communication.
00:45It also said that there was insufficient crew training and awareness about the operation
00:49of the lift dump system.
00:51It highlighted that the aircraft operator also failed to obtain necessary approvals
00:56for non-scheduled air services and comply with Malaysian regulations.
01:01The AAIB urged aircraft operator Jet Valet Sendiam Berhad to enhance training programs
01:07for all crew members and called on the Civil Aviation Authority of Malaysia to review the
01:12current regulatory framework to ensure better oversight of foreign aircraft operated by
01:17foreign licensed aircrew.
