01:00Whether you are a good candidate for liposuction or not
01:02Or is there a risk of any active disease
01:04or if you are an active patient
01:06So this whole consultation is about
01:08checking all the risk factors
01:10So, there is definite risk factors
01:12for any active disease
01:14And there are many lists of things
01:16we have to rule out
01:18before the surgery
01:20Assalam o alaikum
01:22I am Dr. Bushra Akram
01:24and I am an assistant professor
01:26of plastic surgery
01:28We will talk about liposuction and very commonly asked questions and we will try to answer them.
01:38Doctor, what is liposuction?
01:40Liposuction is basically a procedure in which we extract the excess fat of the patient through cannulas or suction machines.
01:54This is done without any major cuts or scars.
01:58Which patients are the best candidates for liposuction?
02:02There is a myth about liposuction which is prevalent that liposuction is a procedure in which the weight is reduced.
02:12Most of the patients come to us with the same problem that we have to reduce weight and for this we want to do liposuction.
02:21This is a myth that needs to be corrected.
02:25Liposuction does not play a role in reducing weight.
02:29Liposuction is done only in those areas where there is excessive fat and we do liposuction to improve the contours of the body.
02:39Liposuction does not play a role in reducing weight.
02:43Excessive fat accumulates in different parts of the body and it looks bad.
02:49It spoils the contours of the body.
02:52To balance this, we remove the excess fat from the body.
02:56This procedure is called liposuction.
02:58Which parts of the body can be liposucted?
03:01Different parts of the body can be liposucted.
03:05We perform liposuction on the stomach, waist, legs and arms.
03:13Some clients have double chin and excessive fat.
03:18We perform liposuction on those areas as well.
03:21What is the procedure to perform liposuction?
03:24First of all, there are details related to the procedure.
03:30More importantly, either you are a good candidate for liposuction or not.
03:36First of all, assessment is important.
03:39When we assess that our client is a good candidate for liposuction,
03:45the next step after the assessment is how to perform liposuction.
03:51In liposuction, there is no big scar.
03:55There are many questions like, will there be a cut or a long scar?
04:01In liposuction, there is a small scar from which we have to pass the cannulas.
04:06Through that, we liquefy the fat and extract it.
04:10This is the procedure.
04:12There are two ways to perform liposuction.
04:15One way is to remove excess fat from a very small area.
04:20This area is such that the patient can do it comfortably without fainting.
04:25We infiltrate local anesthesia and extract the fat from there.
04:31But if the area is such that the local anesthesia is not comfortable for the patient
04:39and the procedure is long and we have to do a combined procedure,
04:46in those cases, we have to perform this procedure through general anesthesia.
04:51Doctor, are there any risk factors for liposuction?
04:54The risk factors for liposuction, as I just said,
04:58first of all, assessment is very important.
05:01Either that patient is a good candidate for liposuction.
05:05Second important thing is that in-clinic consultation is very much important.
05:12Its purpose is to rule out all the risk factors,
05:17medical history, medicines that we need to stop,
05:22or any active disease in which we cannot perform this procedure.
05:27So, assessment before procedure and in-clinic is very important.
05:32In this regard, I would like to say that some patients don't come for evaluation
05:38and they prefer to come for the procedure.
05:43So, this is very important and it's good for the patient.
05:47When you come for assessment,
05:49it's not just that the doctor has to tell you the details about the procedure,
05:54but it is also very important that how fit you are for that procedure
05:59or whether you are a good candidate for that procedure or not.
06:02So, the purpose of all those consultations is to assess your risk factors.
06:08We assess the contraindications of performing the procedure.
06:14So, there are definitely risk factors.
06:17If there is any active disease in which we should not do this procedure,
06:21then we have to rule it out beforehand.
06:24And there are lists of things that we have to rule out before doing the procedure.
06:29So, definitely there are risks and there is a level of Body Mass Index
06:34in which we can perform the procedure.
06:36If there is any active disease or if a patient is using any medicine
06:40and it is very important to stop it for some time.
06:43So, all these things are a part of our consultation.
06:46Doctor, is there any criteria for which patient is the best candidate?
06:53Yes, there is a criteria.
06:55As we were talking about Body Mass Index,
06:58the first myth that we talked about is that it is not a tool to reduce weight.
07:03It is just a contouring procedure.
07:05There is also a range of BMI,
07:07in which we perform liposuction of the patients who fall.
07:12So, this is the criteria of Body Mass Index.
07:15The second criteria is their health factors and whether they are controlled or not.
07:21The third criteria is their history of medication.
07:24And the fourth criteria is the patient's expectations.
07:30There are two categories.
07:31One patient comes and says that this is a bulge of fat and we want to remove it.
07:37Another patient is 5 feet tall and weighs 120 kgs.
07:42That patient comes to us and says that he wants to lose weight and he wants liposuction.
07:47So, that is the scenario where we have to exclude
07:51which patients are falling in our criteria and which patients we have to advise for treatment.
07:58So, definitely there are some criteria set for this.
08:01And according to that, we select our patients.
08:04So, doctor, for the patients who are not the best candidates for liposuction,
08:09is there an alternate surgery for them?
08:11Yes, there is an alternate surgery for them.
08:13There are expectations in it.
08:15Some people come to us and say that the skin of their stomach has become dull
08:20and there is additional fat.
08:22We have a problem getting up and sitting.
08:24And we want to get liposuction done.
08:28So, we have to guide them accordingly.
08:32If they are fit according to our assessment for surgery,
08:36then they are such a candidate in which we advise their abdominoplasty instead of removing fat.
08:42So that their dull skin and loose muscles and all those things
08:46we can deal with them better through a proper surgical procedure.
08:51So, definitely your treating consultant will definitely advise you the best option.
08:58And that is the important thing that you have to ask in the discussion before surgery.
09:04The best way and the best modality for your concerns.
09:08So, definitely the patients who do not fall in the criteria,
09:11we advise them the procedure that is best for them.
09:14So, Dr. Sir, the patients who have diabetes or blood pressure,
09:17can they also undergo this surgery?
09:19The thing is, as we are talking about health factors,
09:22when we hear the name of diabetes,
09:25we think that it will be a 70-year-old male.
09:29It is not like that.
09:30He will have a lot of problems.
09:32If diabetes in a young patient,
09:34and he has done his glycemic control very well,
09:38good exercise,
09:40he has maintained his weight very well,
09:42and that patient comes to us,
09:44definitely we can help in treating that patient if they have any concern.
09:49So, it's like custom-made.
09:52Every patient, we cannot set any rule in which we say that
09:56we can do liposuction in every diabetic patient.
09:59And we cannot say that we cannot do liposuction in every diabetic patient.
10:04So, this is a procedure in which we have to give a custom-made treatment plan.
10:09So, the patients who come to us,
10:11their health issues, their history, their medication history,
10:15keeping all these in mind,
10:17we advise them the best treatment plan.
