Texas "BP" Refinery Disasters:--
Fire and Explosion at the "BP" refinery in Texas, burning and series of explosions occurred in the hydrocarbon workshop of the BP refinery in Texas, USA. Reasons For The Explosions: -

Earlier, it has been reported that the tower's level gauge, liquid level observation hole and pressure control valve have failed , But the device still drove as planned. (02) The management staff of the company and the Texas refinery did not perform effective leadership and supervision responsibilities, did not pay enough attention to the company’s safety culture and major accident prevention system, and the company’s management was ineffective in safety supervision. It did not provide sufficient human and financial resources, and did not establish a safety management model to supervise and evaluate the implementation of safety regulations and operating procedures by employees. (03) The company's management did not pay attention to the hidden dangers of accidents in the Texas refinery. Employees are not supervised to report hidden dangers. (04) The company's shift information communication system is not strict enough, and clear work records are not mandatory to ensure that the information communication between operators is clear. The managers and supervisors of the Texas refinery did not tell The operator should lead the oil product to the storage tank, and the night shift staff did not inform the day shift staff of the working conditions of the device, especially the height of the liquid level in the tower, during the shift. (06) Insufficient manpower, inadequate training of operators, and no special emphasis on the dangers during operation. (07) The company did not establish a good accident investigation and management system (1994~2004, the company’s hydrogenation unit empty tank, there have been eight serious accidents, but only three accidents have been investigated. (08) The vehicle that has not stalled is too close to the device with a risk of explosion, and the management personnel of the company's Texas refinery.
