00:18And to explain what hoarding and hoarding disorder means,
00:23we will now join the psychiatrist, Dr. Kathleen Tan.
00:27Good morning, doctor.
00:30Hello, good morning to all of you.
00:32Thank you for having me this morning.
00:34For our RSP members, what is hoarding and what are the common signs and symptoms of hoarding disorder?
00:46Like what you pointed out, many of our countrymen have symptoms
00:51where they collect things that they don't need and there's a clutter.
00:57So we call this hoarding disorder because we can see that these symptoms
01:04affect our functionality and our capability to be able to live our lives properly.
01:17So for example, hoarding disorder, there are three things that we have to establish.
01:24So one, you have an urge to collect and keep things that you don't need to use.
01:33So it's common to see paper bags that are too many
01:38because we think that we will use plastic bags,
01:42especially bottles and take-out containers.
01:51So we see that it's very, very common.
01:54And then we also see that because of this collecting, it's too much.
01:59So there's also a clutter.
02:01So we can see that it's chaotic inside the house because it's taking up so much space
02:08and yet it's hard for our patients to let go.
02:13And then finally, we also see that their habits affect their function
02:21and their well-being and lifestyle.
02:27Doc, I feel like we're all guilty here because we have moments like this in life.
02:35Doc, what are the possible causes and triggers of hoarding disorder?
02:43Well, you know what?
02:43Before, they considered hoarding disorder as part of obsessive-compulsive disorder
02:49but they saw that it's not the case.
02:52There are explanations that there is a decrease in function in a particular part of the brain.
02:59But overall, we also see that it's not actually that well understood.
03:07But we also consider that there's a genetic component to it.
03:11It's also noticeable that what we see is more of an advance of age.
03:17So possibly, epigenetics could also be a culprit then.
03:23Because if you notice that these particular generations are products of the world war.
03:31So for example, resources are very scarce and your adaptation is to hoard, to keep things.
03:38Because we're stockpiling for the possibility that things may be scarce again
03:46and we also need it eventually.
03:48So it becomes a habit for us.
03:51And then it's carried over from generation to generation.
03:53Because that's what our parents and grandparents are teaching us.
03:59But there are multifactorial components.
04:01But overall, when we're talking about hoarding disorder,
04:07the statistics are high.
04:09They estimate about 3 to 5 percent of the general population
04:14with a lifetime prevalence of 14 percent.
04:16So it's actually quite a common court condition.
04:21But what's concerning is that sometimes these habits are so severe
04:26that it can also cause health problems or hazards.
04:33Like for example, if there's a lot of clutter inside the house
04:39and we're wearing an earthquake belt.
04:43For example, if it's hot and we don't have enough water.
04:49It's also a fire hazard.
04:50It can catch fire and it can also be dangerous.
04:54And then it can also be a falling hazard.
04:57For example, if there's a lot of stuff, it can fall
05:01and it's hard to navigate your way inside the house.
05:06And then there's also a health risk because it attracts pests.
05:09Rodents.
05:10And then for some extreme cases, we've been collecting things that are
05:19very infectious already.
05:23I do recall watching a particular TV series called Hoarders.
05:28And there was an episode where this person was collecting already used diapers
05:32and it piled up in the bathtub.
05:34So it could be extreme measures.
05:36And then it can cause disease and death.
05:39Especially hoarding.
05:41It's not limited to the use alone.
05:44It can also be animals, pets, strays that are rescued.
05:50But unfortunately, they are neglected.
05:53So yeah, it's a very concerning condition actually.
06:07My mom likes to put things next to her.
06:09I like to put less and less so it's always clean.
06:12But here's the thing.
06:13Some of our fellow countrymen are just sentimental.
06:17Yes.
06:17So what's the difference between being simply sentimental
06:21or really fond of collecting and disorder of hoarding?
06:27Well, like what you pointed out when it comes to hoarding.
06:29What we hoard are useless items.
06:34And then it accumulates to significant numbers already.
06:41So how much paper bags do we actually need through the years?
06:45So for example, it's a whole room full of paper bags already that you don't use.
06:50Maybe you have to reconsider.
06:52But for example, we're talking about a collection.
06:55So for example, people collect sneakers, figurines.
06:59That would be a different case.
07:01It's probably displayed properly, organized, and with intent.
07:06And like you pointed out, there is probably some value to it.
07:12But for hoarding, what we usually see is we tend to collect things of insignificant value.
07:20Like you pointed out, pieces of mail that are 10 years old already.
07:25Newspapers that are stockpiling already.
07:30Now what good would it be for?
07:32And I also mentioned that it has to establish the three criteria
07:37that there is a collection of useless things and the inability to let it go.
07:46And then there will be clutter in the house.
07:49So the emphasis is on the word clutter.
07:51And then the third is it also causes dysfunction in our capacity.
07:59It impedes our daily function, our daily lives.
08:14Well, unfortunately for hoarding disorder,
08:17when we talk about hoarding disorder,
08:18I'm talking about the actual psychiatric diagnosis.
08:22Because hoarding can be used as a verb.
08:25Like okay, I'm hoarding or stockpiling canned goods just in case something happens
08:32and I'm not able to refill our groceries.
08:36Although like I pointed out, hoarding has insignificant value.
08:44But talking about the psychiatric disorder itself,
08:47unfortunately hoarding disorder is treatment resistant.
08:51So studies show that although the prognosis is very poor,
08:55but the recommendation is to consider psychotherapy.
08:59So a particular type of psychotherapy that is recommended is cognitive behavioral therapy.
09:04And then another, in terms of medication,
09:08we can also consider antidepressants usually from the family of SSRI,
09:15Selective Serotonin Reuptake Inhibitors.
09:18Again, our expectations are very limited.
09:23Because probably one factor that could make this condition quite difficult to manage
09:32is usually we diagnose it late.
09:34So usually it's advance of age and then we see,
09:39wow, there are so many and the house is so tight and the house is already in a danger zone.
09:46And then sometimes it could be extremes already,
09:49that our patients are already being evicted
09:51because when the landlord saw it, it's already dangerous to be inside the house
09:59and it's very unsanitary already.
10:01So unfortunately, these are our challenges when managing hoarding disorder.
