00:50avoidable if this person had been treated with any form of responsibility whatsoever.
00:56It was lazy and quite frankly we believe that the clinicians involved and the staff involved
01:04need to be held accountable because basically homicide does not occur without problems and
01:09delivery of standard clinical care to patients and he did not receive that and as a result the
01:15consequences have been disastrous, heartbreaking and life-destroying.
