Transcript
00:00 You just got hot outside but you're already red.
00:04 You just ate spicy food but you're already smiling.
00:08 You just exercised but you suddenly have a rash on your body.
00:14 Oh my gosh! What's happening?
00:17 If you keep experiencing that, you might have chronic uticaria or hives.
00:24 Hives or tagulabay in Tagalog is a condition where red, thick, and bulging skin appears whenever the body temperature rises or when you sweat.
00:40 This morning, we will learn about the symptoms and treatment for this condition here in ZanyDoc.
00:49 [Music]
00:55 And now, here in ZanyDoc, we will give a free telemedicine about this condition.
01:00 We will be joined by Dr. Ravia Roderos Galvan, a general physician.
01:05 Dr. Ravia, welcome back to Rise and Shine Pilipinas.
01:07 Good morning.
01:08 Good morning, Ms. Diane, Sir Audrey, and to all our viewers, Rise and Shine to all of us.
01:14 I remember you explained this to me earlier so that I can experience it when the weather is hot.
01:20 You get a cold.
01:21 I supported my friend in a campaign. We were in the heat.
01:25 I got a cold. Can you explain what heat hives are?
01:32 What I experienced is very common.
01:36 Heat hives or what we call cholinergic urticaria is an immune or allergic reaction to heat.
01:47 Because of the heat of the moment, we see it more often.
01:51 What happens when we have cholinergic urticaria or what we call heat hives is that our body reacts to heat.
02:01 That's why we see that we get a cold. Sometimes, we get a cold not only from a cold but also from a burning sensation because of our body's reaction to heat.
02:12 So, it's a bit of a hot skin.
02:15 But what are the risk factors or triggering factors in this condition, aside from the heat, Dr. Ria?
02:21 We mentioned earlier in our video that it can be caused by a lot of things, not just heat.
02:27 Number one, exercise, especially when you exercise under the sun.
02:33 You jog or you walk.
02:35 Yes, because most of us want to be healthy.
02:39 But because of the heat of the moment, our body can react and we get urticaria or what we call hives.
02:47 Another would be spicy food because it triggers a heat reaction in our body.
02:53 For example, you eat spicy food but you're wearing an air-conditioner and your urticaria is triggered.
03:02 That can happen.
03:03 Another would be stress or anxiety.
03:06 Sometimes, when we're stressed because of the heat, that reaction can trigger.
03:16 So, for example, you eat spicy food, you're wearing an air-conditioner, your urticaria is triggered,
03:25 it will manifest in your skin, right?
03:27 Yes, it can.
03:28 Because cholinergic urticaria is a cholinergic response.
03:35 Meaning, there are enzymes or chemicals in our body that react to heat.
03:44 We call it acetylcholine.
03:47 So, when that happens, the theory is that when heat triggers acetylcholine, our body responds.
03:57 The manifestation is actually urticaria or sometimes, our hives or our urticaria are affected.
04:07 So, it's an immune reaction.
04:10 In simpler terms, are we saying that these people are allergic to heat or spicy food?
04:18 We can say that because our body responds to a stimulus or something outside our system.
04:27 Just like allergens, our body responds to heat, that's why we get a fever.
04:35 It's like an allergic reaction.
04:38 So, is it like a simple antihistamine will do, Dr. Bea?
04:42 Actually, that's a good point, Ms. Diane.
04:44 Usually, what we prescribe when we experience cholinergic urticaria or hives is antihistamine.
04:55 Because it's an allergic, it's an immune reaction.
04:58 What cancels our body is antihistamines.
05:04 But other factors or other things can be done also so that we don't have to wait for the antihistamine effect.
05:13 Cold compress, like what we talked about earlier with Sir Audrey, you did a cold compress on his body.
05:20 Yes, so that you can put ice.
05:22 So that you'll be more comfortable.
05:24 So, it's possible.
05:25 Yes, these are the home remedies.
05:26 You put ice because it's a bit itchy.
05:30 It's itchy and itchy.
05:31 Aside from that, are there other home remedies?
05:33 Wear very comfortable clothing.
05:35 So, clothes that are made of cotton or clothes that are fresh on the body is actually important
05:44 so that we have ventilation, so that we're more comfortable.
05:49 Another would be we avoid certain triggers.
05:53 For example, now, because the weather is hot, many of us are complaining.
05:59 We always go to a well-ventilated area, if possible, air-con.
06:07 Because the weather is so hot, we can't avoid it.
06:11 Another would be avoid spicy food and exercise, especially when it's hot.
06:16 Or we exercise under the hot sun.
06:20 We should avoid that.
06:22 I know someone who drinks coffee in the early afternoon because of the hot sun of the Filipinos.
06:28 And then, he goes jogging.
06:30 But, Doc, are there specific areas where these symptoms are usually shown?
06:36 Is it on the hands, on the back, or on the neck?
06:39 Usually, there's no specific area.
06:43 But usually, we can see it on our elbows, on the neck area.
06:48 And one of the things that we should actually pay attention to,
06:54 apart from the itch, because it's an allergic reaction,
06:58 we can actually have anaphylaxis if our body's response is too much.
07:04 That's why it's important that when we feel it, we should drink antihistamine
07:09 so that it won't go back to complications like anaphylaxis or severe allergic response.
07:19 So that's the worst thing that could happen if you neglect it.
07:23 But are there different types of heat hives, Doc Villa?
07:30 Usually, heat hives are just a common response of our body.
07:36 It's like an allergic response to heat.
07:38 But there are also what we call heat rash, which is another thing.
07:42 When our sweat glands are trapped, that's another thing.
07:48 But usually, heat hives can actually be triggered by a lot of allergic reactions such as asthma.
07:56 So, it's prone to have asthma, allergic rhinitis, to have heat hives or to respond to heat.
08:06 Okay, Doc, aside from having heat rash, are there other symptoms that can be experienced by heat hives?
08:15 So that you can control it right away?
08:17 That's a very good question.
08:19 Because heat hives can have a burning sensation, as I mentioned earlier.
08:24 And then sometimes, especially those who might have other pre-existing conditions like asthma,
08:31 they can have shortness of breath, anaphylaxis or anaphylactic response.
08:43 So, not only because it's a symptom, but there are other symptoms that can actually tell you that you're experiencing cholinergic or tic aria.
08:55 So, it's not just because of the symptoms that we expect.
09:02 Are there certain types of foods that are more vulnerable to have this, Doc?
09:06 Usually, we see this in very active people.
09:10 I see this in my patients between 10 to 30 years old.
09:14 So, it's a wide range of age groups.
09:17 Probably because they're more active, probably also because they're more prone to be exposed to heat.
09:25 And that's the reason why you can see more cases like that in this age group.
09:31 So, there are a lot of tips and recommendations, Doc, via so that we can avoid it.
09:38 But lastly, do you have any additional reminders for our viewers regarding this so that we can avoid this condition?
09:45 Is it the lack of water?
09:47 If you're self-hydrated, is that okay?
09:50 I always tell my patients that prevention is always better than cure.
09:57 Especially with the effects of the weather.
10:01 So, always remember to stay hydrated, always keep yourself ventilated, and let's be careful with the effects of the weather.
10:10 Thank you for sharing all the information with us.
10:13 We have a free telemetry with Dr. Via.
10:18 Thank you very much.
10:19 Thanks, Doc Via.
10:20 Thank you. Good morning to all of us.
