00:08 So for some people who are misfortunate enough in order to have problems of no sperm count
00:15 This is a condition called Azoospermia
00:18 In fact there are two reasons why it happened
00:26 There's a quiet reason and there's a congenital reason
00:29 Let's deal with congenital reasons first
00:31 For example if you are unfortunately born with genetic conditions such as Klinefelter condition
00:38 Then the genetic material in your body will actually inhibit the spermatogenesis
00:45 So under that circumstance it's impossible to find the sperm at all
00:50 There's another genetic condition that actually will result in somebody producing sperm
00:55 But the sperm is not delivered to the outside world
00:59 And that's a condition called Congenital Bilateral
01:03 Because it's congenital in both sides
01:05 Aplasia of Vastifrans
01:08 So if you imagine that the tube that delivers the sperm from the testicle to the outside world
01:14 Is actually broken because of genetic condition
01:18 And that condition actually will not be visible or will not have any symptom at all for an individual
01:25 Until they are trying to have a baby
01:27 And it's noticed that they do not produce any sperms at all in the ejaculate
01:32 When we assess their hormone level which is the testosterone and FSH
01:39 We notice that these are all normal because the only thing that's abnormal is the delivery system
01:45 So under that circumstance there are ways for us to actually retrieve the sperm directly
01:51 In order to create test tube babies
01:54 So those are the congenital reasons why people have Azoospermia
01:58 However there are other reasons why people acquired Azoospermia
02:03 For example, infections
02:06 If you are unfortunate enough when you are a kid you have infection of Epididymal Orchitis
02:12 Or Mumps Orchitis and that infection itself destroy the Spermatogenesis
02:18 In addition to that other infections such as sexually transmitted infections
02:24 Like Gonorrhea or Chlamydia will block off the tube that produce sperms
02:29 So obviously it's quite different between the blocking of the tube and also the destruction of the sperms
02:36 If the destruction of sperm producing cells then you will not be able to retrieve any sperm for test tube baby
02:44 However if somebody has undergone vasectomy or somebody who actually had sperm duct obstruction
02:50 Due to sexually transmitted infection
02:52 Then there is a hope that we will do something called MISA
02:56 Microscopic Epidermal Sperm Aspiration or TC
03:01 Testicular Exploration Sperm Aspiration or Sperm Extraction
03:08 Then we will be able to get that sperm to create that dragon baby for you
03:13 If you had made the decision to have a vasectomy
03:16 So therefore you are sterile and not being able to produce natural sperm to create a dragon baby
03:24 But you change your mind and thinking about reversal of vasectomy
03:28 Or you are simply interested in a vasectomy and want to know the possibility of the reversal
