INVERCARGILL, NEW ZEALAND — A coroner's report has revealed the tragic details that led to the 2014 death of a New Zealand road worker who slipped in his own vomit, fell from his vehicle and was then crushed by it.

The New Zealand Herald reports on coroner David Crerar's findings: Garry Voorkamp, 53, had been feeling unwell on September 24 of last year. He had complained to workmates of a headache during a mid-morning work break and the painkiller he had taken was having no effect. Voorkamp brushed off suggestions he could take the remainder of the day off if he chose.

Back at the controls of the road grader he was operating, his state worsened, and while driving, he vomited. Crerar determined Voorkamp slipped on his own vomit while trying to exit the moving vehicle and fell out of the left-side door. Voorkamp fell into the path of the moving road grader's rear wheels. It's not known whether the Voorkamp was conscious when the wheel rollover him, but the head injuries he sustained ended his life.

Having thought Voorkamp had pulled over for some reason, coworkers investigated and found their friend lying dead between the wheels.

Voorkamp is said by colleagues to have been highly safety-conscious, and Crerar was unable to determine why Voorkamp had tried to exit a moving vehicle.
