00:57John Eric Hexham was just 26 when he died fooling around on the set of the CBS TV series
01:03Cover Up.
01:09An actor and model with a promising career ahead, Hexham decided to play Russian roulette
01:14in between takes with a gun used in a previous scene, which was loaded with blanks.
01:20The actor was unaware that firing blanks at close range could have disastrous effects,
01:26put the gun to his temple, pulled the trigger, fractured his skull, and caused massive hemorrhaging.
01:36He was declared brain dead after six days of surgery, and his mother gave permission
01:40to switch off life support and donated Hexham's organs.

02:20The mind behind a hair-raising human pyramid in which seven acrobats formed three tiers
02:25and cycled across a wire, Wallenda made one walk too many in 1978.
02:31The patriarch of the Great Wallendas was best known for the creation of their three-level
02:35human pyramid.
02:36It was also their most dangerous act.
02:38The then-73-year-old fell ten stories during a stunt between the two towers of the Condado
02:44Plaza Hotel in San Juan, Puerto Rico.
02:47Wallenda had famously described being on the wire as life, and the rest is waiting.
02:58It was a balancing act that eventually, horribly, proved the death of him.

03:05Jerome Irving Rodale's death came with particularly dark irony.
03:11The writer, magazine editor, and health food guru for America passed away on The Dick Cavett

03:20Although technically not a performance, he had just finished his interview and was sitting
03:24to one side while another interviewee, columnist Pete Hamill, spoke to the show's host.
03:30During Rodale's earlier section, the health enthusiast had been discussing the benefits
03:34of a natural diet and lifestyle, reportedly quipping that he had, quote, decided to live
03:39to a hundred, and that he'd, quote, never felt better.
03:43Just a few minutes later, he suffered a heart attack and was dead at 72.

03:58As guitarist for the 70s blues rock group Stone the Crows, Les Harvey's career was just
04:03taking off when he died on stage at a show in Swansea, Wales.
04:07Stone the Crows, who were managed by Led Zeppelin's Peter Grant, were fresh off of their third
04:12album, Teenage Licks, when Harvey was doing a sound check before a gig in May 1972.
04:18On a rain-soaked stage, Harvey touched what turned out to be an ungrounded microphone
04:22and was electrocuted on the spot.
04:25The band never truly recovered, although a fourth album was released, and Harvey is forever
04:31listed as part of the famous 27 Club.

04:49The American actor Vic Morrow was killed with two child actors, Micah Din Lee and Renee
04:54Shin-Yi Chen, while shooting a scene for 1983's Twilight Zone the movie.
05:03The trio died when pyrotechnics caused a low-flying helicopter to crash on top of them, and Morrow
05:09was decapitated by the rotor.
05:12The incident brought unprecedented legal action against the film crew, as director of the
05:16sequence John Landis and others were tried on charges of manslaughter, but acquitted.

05:35As the son of martial arts legend Bruce Lee, Brandon Lee was busy following in his father's
05:40footsteps when a freak on-set accident took his life in 1993.

06:01shot by a faulty Prop 44 Magnum after an improperly deactivated cartridge became lodged in the
06:07barrel.
06:10Fellow actor Michael Massey unknowingly fired the shot, during a scene in which Lee's character
06:14walks in on his fiancée being beaten by thugs.
06:21The movie, which was days from completion when tragedy struck, was finished with the
06:25support of Lee's family, and with Chad Stahelski stepping in as Lee's double.

06:35Professional wrestlers face risks every time they enter the ring.
06:41Owen Hart was one of the WWF's top talents, heading into Over the Edge, a pay-per-view
06:46event on May 23, 1999.
06:53Hart, performing as the Blue Blazer at the time, was being lowered into the ring superhero
06:59style when a quick-release mechanism malfunctioned and he fell 78 feet, landing chest-first on
07:05the top rope.
07:12Several attempts were made to revive him, but he was killed by blunt force trauma.
07:17Controversially, the show continued, although the actual incident was not aired.
07:33While Owen received treatment, it was commentator Jim Ross who had the unenviable task of assuring
07:39viewers at home that Hart's incident was not an act.

07:59As one of Britain's best-loved and respected comedians and magicians, Tommy Cooper was
08:04an icon of his time.
08:14Known for his red fez and his Just Like That tagline, he was a gifted showman, but just
08:20like that, he was gone.
08:24Millions of TV viewers watched in April 1984 when Cooper collapsed midway through his act
08:29on the variety show Live From Her Majesties.
08:32Given his penchant for slapstick, the audience roared with laughter, thinking Cooper's very
08:37real heart attack was all part of the show.
08:40But as those backstage realized what had happened, the curtain was drawn on Tommy one last time.

08:55An Australian treasure and an international star, Steve Irwin boldly went where most wouldn't
09:02to bring the world's most dangerous animals to our TV screens.
09:06Known as the Crocodile Hunter, he got up close and personal with crocs, dragons, lizards,
09:12snakes, sharks, pretty much everything.
09:15But Steve's last subject was a six-and-a-half-foot stingray, which he was filming just off the
09:19coast of Port Douglas, Queensland.
09:25The animal unexpectedly turned on Irwin during the shot, and stabbed its barb wildly into
09:30his chest.
09:31Irwin initially suspected a punctured lung, but the attack had pierced his heart, and
09:36he died of blood loss before the crew could return to land.

10:32We've seen sudden deaths, accidental deaths, and accidental killings, but Dimebag Daryl
10:44was murdered while performing.
10:55Damage plan was just moments into their set, when ex-Marine Nathan Gale shot Daryl Abbott
11:00three times in the head with a 9mm pistol, before killing four more people, and taking
11:06a hostage.
11:07The carnage only ended when a police officer came from backstage, and shot Gale dead.
11:12Theories on Gale's motive include anger at how Dimebag's former band Pantera had broken
11:17up, and claims that Pantera had stolen some of Gale's songs.
11:21However, police never officially recognized a reason for Dimebag's senseless killing.
